Social Worker, £32ph, Hospital Discharge, Southwark
Pertemps are recruiting for a qualified and experienced Social Worker to work within the Transfer of Care Team (TOC) South. This role is predominantly place-based in the hospital discharge team and visiting patients on the ward for complex discharge assessments and dedicated discharge planning, including mental capacity assessments and discharge planning meetings with medical colleagues, therapists and/or social care professionals and families.

This post requires 3-4 days per week office presence near the hospital, generally 1 day/week working from home for write-ups.

Roles & Responsibilities:

• To provide a comprehensive, effective, integrated, systemically informed social work service, within Southwark’s practice framework, managing complex cases.
• To deliver excellent social work practice, assessing complex need, planning, and working purposefully with families, support networks and other professionals to manage risk, safeguard children and promote positive outcomes.
• To provide a social work service which is both relationships based and which responds in a timely manner within statutory guidelines.
• To work collaboratively with the team, supporting the development of best practice and confident practitioners through coaching and mentoring of students, newly qualified social workers and less experienced practitioners.
