We are recruiting for motivated qualified social worker to join our busy team. This role is with one of the UK’s highest performing local authorities, serving as one of the best places to live and work in.
 
This is a qualified role requiring you to be SWE registered and have relevant experience within adult’s services.
 
Social Worker role within Reablement Team. We are community based multidisciplinary team. We work with customers, who have potential to regain their independence. we work closely with health teams. 
 
We also complete assessments of needs, long term care plans, MCA & Best Interest, Risk Assessments, Safeguarding.
 
The team is based in Brent Civic centre, staff are expected to work in the borough min 2 days per week at present. the role involves frequent home visits.
