Adults Social Worker – Lincolnshire

Rate: up to £35 per hour
 
We are recruiting for motivated qualified social worker to join our busy team. This role is with one of the UK’s highest performing local authorities, serving as one of the best places to live and work in.
 
This is a qualified role requiring you to be SWE registered and have relevant experience within adults services.
 
Senior Social Workers required to work onsite at a busy acute hospital in Boston, Lincolnshire. You will be completing proportionate assessments to support a person’s discharge from hospital in a timely manner.
Utilising Home First principles and strengths-based practice, you would be supporting the individual, working with their families and carers, and liaising with multi-disciplinary teams to support discharge to the person’s own home where possible.
 
Being able to work collaboratively with Health and the Voluntary sector is a must. Knowledge of the Care Act, Mental Capacity Act, Continuing Healthcare and Safeguarding practice are essential to the role as workers for these positions will support junior members of the team. This is a 7 day service - working as part of a rota at weekends is included in the job role.
