Our client is urgently looking for a Social Worker for Adult Service


To manage a caseload of clients to provide appropriate assessment, care plans and service delivery packages for clients in accordance with the Departmental Procedures.

POST HOLDER’S DUTIES

•To undertake client assessments, to agree appropriate care packages with the client and family/carers as required and to ensure that, where appropriate, assessment of financial contributions are made
•To maintain case records and to adhere to procedures and policies as defined by the Council and the Department, and as required by Statute
•To ensure on-going work, care plans and care packages are reviewed in accordance with Departmental policies, contractual agreements and in liaison with appropriate colleagues, partner agencies and service providers
•To undertake investigations and prepare reports for panels, case conferences and courts as required
•To attend and participate positively in team meetings to promote and integrate responses to service users and to provide best value services
•To attend supervision sessions as required and to follow guidance and advice given
•To provide cover for the work of other team members and to participate in the maintenance of a Duty System as required.
CANDIDATE REQUIREMENT

Recognised Social Work qualification approved by General Social Care Council

Previous experience working within Adult Service

Casework, assessment, advice, intervention, counseling and advocacy skills

Working as a team member making positive contributions

Good verbal, written and IT skills

Communicating clearly and effectively with vulnerable clients in a non-judgmental, non-discriminatory way

Work within a changing and complex environment providing services to vulnerable and often high risk clients

Manage a caseload of varying complexity and to prioritise competing demands

Formulate care plans and risk assessments and maintain clear and effective caseload reporting

Full clean driving license and daily use of car
