Network Health and Social Care are currently looking for Temporary Full Time Qualified Social Worker to join Adult Safeguarding team to work within Plymouth City Council.
You will be expected to have knowledge and previous work skills in the following areas:
" Undertaking holistic assessment and determining priority needs of clients in accordance with departmental policies, standards and guidelines.
" Initiate appropriate statutory action to protect vulnerable adults identified as at risk.
" To carry out any related preventative and therapeutic activity appropriate to the Social Worker role that promotes and maintains the independence and protection of vulnerable adults.
" Manage a caseload to ensure clients receive care to meet their assessed needs, within the team's care management budget.
" Monitor the standards and quality of care provided to clients through service contracts to ensure this meets the standards specified within this contract and conforms to policies.
" Undertake regular reviews in accordance with policies.
" Participate in a day duty rota system and other shared team responsibilities.
" Contribute to decision making on client priority needs. Maintain records and comply with financial and administrative procedures
" Provide information and advice on services, charging policies and benefits.
" Develop multi-agency working relationships with health and other colleagues and work with vulnerable adults, their carers and their communities to promote independence
