Job Description
Advanced Practitioner Neighbourhood Transformation Team, Greenwich
Pay rate - £34-36 per hour.
To lead and support staff through a period of organisational transformation, ensuring a smooth and positive transition into redesigned, locality-based teams. The role will champion a culture of continuous improvement and person-centred practice, fostering a strong, neighbourhood and strength-based values-driven approach to care and support. You will work closely with managers and frontline staff to embed new pathways, protocols and ways of working, ensuring they are understood by staff, adopted and sustained in day-to-day practice. A key focus will be on strengthening professional confidence, promoting integrated neighbourhood working, and enabling teams to deliver coordinated, high-quality support to residents.
