Islington Council is seeking motivated and compassionate Adult Social Care Case Managers to join its Adult Social Care service. This is an exciting opportunity to support adults, carers and families by coordinating care, promoting independence and delivering person-centred, strengths-based interventions. 
As a Case Manager, you will work within a multi-disciplinary team alongside social workers, occupational therapists and other professionals. You will manage a varied caseload, undertake assessments, support planning and reviews, coordinate services, and connect residents with community-based resources to help them live independently and achieve positive outcomes. 
£28.91 per hour
