A personal advisor (PA) plays a crucial role in supporting young people as they transition out of care, helping them develop the skills and resources needed for independent living.
Assessment and Planning: Personal advisors conduct needs assessments to understand the individual circumstances of care leavers. They help create a pathway plan, which outlines the support and resources needed for a successful transition to independence
Support and Guidance: PAs provide ongoing support, offering advice on various aspects of independent living, including housing, education, employment, and health services. They ensure that care leavers are aware of their entitlements and rights
Regular Meetings: Personal advisors are required to meet with care leavers regularly, typically at least once every two months. This ensures that the young person receives consistent support and that any changes in their situation are addressed promptly
Advocacy: PAs act as advocates for care leavers, helping them navigate systems and access services. They can also assist in resolving any issues that may arise during the transition process
Continued Support: After leaving care, personal advisors continue to provide support until the individual turns 25, ensuring that they have the necessary resources to thrive independently
