Adult’s Senior Social Worker, Community Team, West London 
Pay rate £34 to £36 per hour 
Contract role, Community Adult Social Care Team 
 
Pertemps are recruiting for experienced Adult’s Social Worker in West London 
This to work in a Community Adult Social Care Team in the Uxbridge area. 
Key Responsibilities 

• Manage a caseload within a community adult social work setting. 

• Undertake Care Act Assessments, reviews and support planning. 

• Complete Mental Capacity Assessments and Best Interest decision-making. 

• Lead and contribute to Adult Safeguarding enquiries, including Section 42 investigations. 

• Undertake Section 117 reviews and contribute to multi-agency planning. 

• Work collaboratively with health professionals, providers, families and partner agencies. 

• Promote independence, strengths-based practice and positive outcomes for residents.
