Job Description
Adult’s Senior Social Worker, Front Door Service, West London
Pay rate £35 per hour
Contract role, Adult Single Point of Access Team (SPA)
Pertemps are recruiting for an experienced Adult’s Social Worker in West London
This is to work in a Single Point of Access Team
We are looking for an experienced and motivated Senior Social Worker to join our Adult Social Care Single Point of Access (SPA) Team.
The SPA Team provides the first point of contact for Adult Social Care, primarily supporting adults who are new to the service or who do not currently have an allocated social worker.
This is a fast-paced duty and triage service, with a strong focus on assessment, risk management, crisis intervention and short-term stabilisation. The role is varied and requires practitioners who are comfortable responding to urgent situations, making timely professional decisions and managing competing priorities.
The team predominantly works with older adults, including people living with dementia and physical disabilities, as well as adults with other care and support needs who do not meet the criteria for specialist services such as Mental Health or Learning Disability teams.
Key Responsibilities
Triage and assess new referrals into Adult Social Care.
Undertake timely and proportionate assessments of presenting needs and risks.
Respond effectively to urgent situations and emerging crises.
Identify immediate safeguarding, welfare and care and support needs and take appropriate action.
Undertake focused, short-term interventions to stabilise situations and reduce immediate risks.
Apply professional judgement to determine appropriate next steps and identify whether ongoing social work involvement is required.
Work collaboratively with individuals, families, carers and a range of internal and external professionals and agencies.
Coordinate appropriate interventions and services to meet identified needs.
Ensure clear, accurate and timely recording of assessments, decisions and actions.
Facilitate appropriate transfer to locality teams where longer-term social work intervention is required.
