Qualified Adult Social Worker – Integrated Urgent Care Community Team – Tameside
Paying £30 per hour 

The Talent Crowd are recruiting for an experienced Adult Social Worker to work in an Integrated Urgent Care Community Team in Tameside.
There is plenty of hybrid working available.
You will take the lead on supporting adults to live well at home. You will be part of a friendly and supportive team and your work will include managing a caseload, Care Act Assessments, Support Plans, Mental Capacity Assessments and Best Interest Decisions and signposting to internal and external services where necessary to enhance wellbeing and independence.
