Vitalis are recruiting for experienced Social Worker to work in a Hospital Team in Blackburn Council.
We are seeking an experienced and motivated Hospital Social Worker to join a busy Discharge to Assess (D2A) team, supporting BwD residents within assessment beds across the local area. This is an excellent opportunity for a skilled practitioner who is passionate about promoting patient independence and ensuring safe, timely discharges from hospital settings.
You will work collaboratively with health professionals, patients, families, and community services to assess ongoing care and support needs following discharge from acute hospital care.
