Senior Adults Transformation Programme Manager – East London            
Rate: up to £32.29 per hour
 
We are recruiting for a Senior Adults Transformation Programme Manager will play a crucial leadership and delivery role in designing and implementing an Adults Transformation programme. The programme will focus on maximising independence and delivery of budget savings and cost avoidance activity. Setting the foundations for a transformed Adult Social Care system, the programme manager will lead the delivery of: an OT-led review post-discharge; design of a high-quality reablement function; reduction and delay in long term care packages; and review of Mental Health provision. Building on these immediate activities, the Senior Programme Manager will support the SRO and service to develop and design a financially sustainable operating model for Adult Social Care and the broader health and social care system.
 
The purpose of this role will be:

• Provide direct programme management leadership to the Adult Social Care Transformation Programme, responsible for driving delivery of programme activity
• Design an ambitious programme which meets the Council’s ambitions to reduce inequality and develop a financially sustainable model
• Using their subject matter expertise, lead the analysis, design and recommendations for financial and demand modelling to support this
• Ensure best practice and innovative approaches are built into the programme, leading the design and development of a preventative practice model
