SEND Preparing for Adulthood Case Officer (14–25) – Newham
Location: Newham (Hybrid – Office attendance every Wednesday is mandatory)
Rate: £282 per day 
Contract: Full-time

We are currently recruiting for an experienced Preparing for Adulthood Case Officer (14–25) to join Newham's SEND service.

This is an excellent opportunity to support young people aged 14–25 as they prepare for adulthood, helping them achieve positive outcomes through effective transition planning. You will manage a caseload of Education, Health and Care Plans (EHCPs), leading annual reviews and coordinating person-centred planning that promotes independence, further education, employment, training, and community inclusion.

Working closely with young people, families, education providers, adult services, and partner agencies, you will ensure that EHCPs remain outcome-focused and support successful transitions into adult life.

Key Responsibilities
  • Manage a caseload of young people aged 14–25 with EHCPs.
    • Lead annual reviews and Preparing for Adulthood planning.
    • Develop outcome-focused, person-centred EHCPs.
    • Work collaboratively with families, schools, colleges, adult social care, health professionals, and external agencies.
    • Support young people to access education, employment, training, independent living, and community opportunities.
    • Ensure statutory timescales and SEND legislation are met.
    • Maintain accurate records and provide high-quality case management.
Requirements
  • Experience working within a statutory SEND service.
    • Strong knowledge of the SEND Code of Practice, EHCP processes, and Preparing for Adulthood pathways.
    • Experience managing annual reviews and transition planning.
    • Excellent communication, organisational, and partnership-working skills.
    • A person-centred approach with a focus on achieving positive outcomes for young people.
