We are currently recruiting for an experienced Adult Social Worker to join the Older People Team in Hertfordshire.
The role will involve:
- Completing Care Act assessments and reviews
- Developing person-centred care and support plans
- Safeguarding and managing complex cases
- Working with individuals aged 65+ and their carers
- Liaising with health, social care and other partner agencies
- Promoting independence, choice and positive outcomes
- Social Work England registration
- Relevant adult social care experience
- Experience working with older people
- Strong knowledge of the Care Act, safeguarding and assessment processes
- Full UK driving licence
