Pay rate - £33.67 per hour.  

• Proven experience using Mosaic case management system 

• Strong background in managing complex social care cases  

• In depth understanding of the Care Act 2014 and its  statutory requirement including  eligibility ,  assessment  and care planning  

• Demonstrated ability  to carry out person- centred assessments
