Job Description
Adult Social worker, Hospital Pathway Team, Sutton
Duties includes: - Carrying a case load. - SW will be allocated Safeguarding cases to triage, Safeguarding Adults enquiry planning, triage, arranging multi disciplinary meetings, - Completing Risk assessments in community , Nursing homes, care homes. - Carrying out Mental capacity assessment and best interest meeting for long term care and support arrangement. - Carrying out Continuing health care assessment, jointly with professionals from ICB. - Carrying out Care act assessment. - Liaising with other professionals and residents family members. Liaising with housing, - Carrying out Court of protection application for deputyship, Sec 21 A procedures for the allocated cases. - Carrying out Duty responsibility 2 days a month. - The role is hybrid working 3 days office based at Civic offices in Sutton, and 2 days home working.
