Hospital Social Work Team. 
Experienced agency SW supporting hospital discharges under the Discharge to Assess model, applying the 3 Conversations Model on Eclipse. The team is hybrid working, attending the office at least once a month as well as working from home. The team works 6 out of 7 days including Saturdays and Bank Holidays. Ideal candidate will be a car driver. 
 
 
Hybrid: office base - New Cross Hospital at least once a month as well as working from home. 
  
There is plenty of hybrid working available.
