Job Description
Adult Social Workers, Learning Disabilities Team, Wolverhampton
Pay: £31.97 Umbrella per hour
• To provide high quality and effective social work practice to respond appropriately to adults with care and support needs and carers in a timely manner, in line with statutory duties and confidently working under relevant legislation and best practice.
• Through meaningful conversations determine need and risk, plan and deliver outcome focused intervention to safeguard and promote wellbeing and support in a strengths based way, centred around people’s idea of a good life, supporting them to live as independently as possible
• To work collaboratively with internal colleagues such as Early Support, external partners, commissioning, providers and others to assess and provide appropriate interventions and support the best outcomes for adults with care and support needs and carers
• To directly support people in situations where there may be varying levels of complexity, risk, challenge and uncertainty in line with the capability of a Social Worker with a minimum of one year’s post qualification experience
• Maintain accurate, up to date records and be responsible for handling sensitive and confidential information
• Develop creative care and support plans which promote wellbeing and outcomes for people, with consideration for providing value for money and best use of resources. There is accountability for interventions that require expenditure through the department or independently
