Vitalis are recruiting for an experienced Adult’s Social Worker in South London
This is to work in a Health Liaison Team
About the Role:
· The Senior QSW (Health Liaison Worker) will be responsible for working directly with adult clients, health professionals, and care teams to coordinate care, facilitate communication, and ensure the delivery of appropriate services.
Responsibilities:
· Act as a key point of contact between health services, social care teams, and adult clients to ensure seamless care coordination.
· Conduct thorough assessments to identify clients’ needs, risks, and eligibility for support.
Develop personalized care plans tailored to individual client requirements.
· Facilitate effective communication among multi-disciplinary teams to promote client well-being.
· Support the delivery of services that enable independence and improve overall health outcomes.
· Maintain accurate records and ensure compliance with safeguarding, confidentiality, and data protection policies.
· Lead case reviews and contribute to team discussions on complex cases.
Provide advice, guidance, and support to less experienced colleagues within the team.
