Social Worker – Merseyside – Hospital Discharge

Social Worker – Wirral – Competitive, Adult Hospital discharge team
This role involves supporting the delivery of health and social care functions for adults/carers across Wirral. You must have two years’ experience have completed your ASYE and be registered with the SWE. You will ideally have experience working with vulnerable and older adults. You will be working to effectively discharge adults from hospitals to home care.
