Job Description
Job title: Community Occupational Therapist
Pay Rate:£34
Job location: South Lakes (Kendal and surrounding areas)
Assignment Length: Initially 3 months, with likely extension until permanent post is filled
Service Area/Team: South Lakes
37 hours per week
JOB DETAILS
Geographical area required to cover; Kendal, Sedbergh, wider South Lakes area, and occasional support into Penrith if required.
Car Driver required due to the rural nature of the area and community-based working.
Caseload information
Varied and dynamic community caseload, some complexity including:
Hybrid working
Flexible approach; however, ideally minimum 3 days office/community-based to support team collaboration and service delivery.

· Working collaboratively within a multidisciplinary team

- Provision of equipment and adaptations
- Seating assessments
- Functional assessments within service users’ homes
- Supporting hospital discharge and prevention of admission

Details of the work to be carried out:
The successful candidate will:
- Manage a community-based OT caseload, prioritising risk and need
- Complete holistic assessments to support independence and wellbeing
- Undertake moving and handling risk assessments and interventions
- Recommend and arrange equipment, adaptations, and seating solutions
- Work closely with health colleagues, social care teams, reablement and partner agencies
- Maintain accurate and timely case recording and documentation
- Participate in supervision, and service development activities.
