Job Description

The Community Rehab Service delivers the Portsmouth reablement offer via an integrated team across Portsmouth City Council and Hampshire and Isle of Wight Healthcare. The service goal is to support people after a stay in hospital, a fall, a medical crisis, or a change in their health, to regain independence and prevent health deterioration.

Our Aim is to assist people of all ages with physical, mental and social impairment to optimise their functional independence.

We are looking for an experienced Occupational Therapist to join us on a temporary basis to support the team in completing assessments, setting patients goals, providing tailored plans and strategies to manage daily tasks, educate and support reablement practitioners, as well as patients and their families, and work collaboratively across health and social care to ensure where ongoing needs are identified these are met in the most appropriate way.

Based at St Marys campus with regular travel across Portsmouth to patients home addresses.
• You will be working with people who have the most complex and complicated needs
• You will work primarily with disabled people and their carer's in their own homes, assessing needs and implementing agreed action plans to facilitate choice and control in their lives and enable participation in the community.
• You will be assessing, advising on and adapting methods of carrying out daily activities to support individuals in managing their disability and promoting independent living and Wellbeing.
• You will use assistive technologies and design and construction methods to improve, restore and attain occupational performance.
• You will recommend, and provide where appropriate, specialised equipment and demonstrate and give guidance to clients and carers in the use of equipment.
• You will provide support and advice to clients and their families/carers in their adjustment to disability and long-term conditions.
• You will be working independently and be responsible for managing your own caseload and maintaining case records within an environment offering a high level of supervision and support.
• You will be fully conversant with statutory legislation and guidance and demonstrate that you implement this in your day-to-day practice and have substantial experience as a practitioner.
• You will understand the importance of safeguarding policy and procedures and integrate and implement these into your work.
