Pay Rate: £38.86 per hour
Pertemps are recruiting an experienced Occupational Therapist to join a Discharge to Access (D2A) service in Slough. This is a hybrid role offering flexibility alongside impactful frontline work.
Key Responsibilities:
- Complete timely and proportionate Occupational Therapy assessments to support safe hospital discharge
- Facilitate Discharge to Access pathways, promoting independence and recovery at home
- Recommend equipment, adaptations and short-term interventions to reduce reliance on long-term services
- Work collaboratively with health, social care and multidisciplinary teams
- Contribute to care planning that prevents delayed transfers of care and unnecessary admissions
- Qualified Occupational Therapist
- HCPC registration
- Experience within Discharge to Access, hospital discharge or community-based adult social care
- Strong assessment, decision-making and risk management skills
