CHC Lead Social Worker – Adults
Location: Southend-on-Sea
Salary: £40,444 – £46,579 per annum
Hours: Full-time, 37 hours per week
Working Arrangements: hybrid role, minimum of 2 days per week on site.
Contract: 12-month fixed-term contract

Pertemps Social Care are currently recruiting for a CHC Lead Social Worker to join Southend City Council’s Adult Social Care service on a 12-month fixed-term contract.

As a CHC Lead Social Worker, you will:

• Attend and actively participate in Decision Support Tool (DST) meetings
• Ensure strong representation of the social care perspective
• Provide appropriate professional challenge and support where required
• Contribute to fair, evidence-based decision-making
• Act as a consultative resource for social work colleagues on CHC matters
• Provide advice and guidance to support best practice and consistency across Adult Social Care
• Work closely with health and social care partners and professionals
• Report to the Level 9 CHC Lead
• Support colleagues with CHC-related queries, processes and practice

About You:

• A recognised Social Work qualification
• Current registration with Social Work England (SWE)
• Completed ASYE
• Current experience working within Adult Social Care
• Strong knowledge and experience of NHS Continuing Healthcare (CHC) processes
• Knowledge of relevant legislation and regulations
• Experience attending or contributing to CHC/DST processes
• Strong assessment skills, including carers’ assessments
• Ability to provide professional support and guidance to carers
• Excellent communication and relationship-building skills
• Ability to work effectively with health and social care professionals at all levels
• Eligibility to work in the UK
