We are currently recruiting for an experienced Adult Social Worker to join a team based in Bournemouth East, Boscombe.
This is initially an office and community-based role, with candidates required to be available to work from the office Monday to Friday.

Key responsibilities:

• Undertake Care Act assessments and reviews
• Work with adults with long-term conditions
• Complete safeguarding assessments and interventions
• Work with adults experiencing self-neglect
• Manage complex cases and promote positive outcomes
• Undertake Court of Protection work
• Work collaboratively with health, social care and other partner agencies
• Develop person-centred care and support plans

Requirements:

• Social Work England registration
• Experienced and qualified Adult Social Worker
• Experience working with adults with long-term conditions
• Strong safeguarding experience
• Experience working with self-neglect
• Court of Protection experience
• Strong knowledge of Care Act assessments and reviews
• Ability to work in the office Monday-Friday initially
