Adult’s Social Worker, LD Team, West London
Pay rate £32.72 per hour
Contract role, Community Learning Disability Team
Pertemps are recruiting for an experienced Adult’s Social Worker in West London
This is to work in a Learning Disability Team in Ealing
Key Responsibilities
Strengths-Based Reviews and Assessments
Conduct Care Act reviews and reassessments for adults with learning disabilities living in the community.
Apply a strengths-based and person-centred approach, identifying individual skills, capabilities, networks, and community resources that can support independence.
Ensure individuals are actively involved in decision-making regarding their care and support arrangements.
Assess eligible needs in line with the Care Act 2014 and local authority policies.
Analyse current support arrangements and determine whether outcomes continue to be met effectively.
Identify opportunities to maximise independence and reduce reliance on formal support where appropriate.
Develop outcome-focused support plans that promote wellbeing and community participation.
Community and Partnership Working
Work collaboratively with individuals, families, advocates, providers, housing services, health professionals, and voluntary sector organisations.
Support individuals to access education, employment, volunteering, leisure opportunities, and community-based activities.
Promote social inclusion and meaningful occupation to reduce social isolation.
Build effective relationships with local community resources and services that enhance independence and quality of life.
Care Planning and Resource Management
Review commissioned packages of care to ensure they remain proportionate and provide value for money.
Consider alternative support arrangements and community-based solutions before recommending increases in funded services.
Prepare recommendations regarding care packages, direct payments, supported living arrangements, outreach support, and residential placements.
Present cases and recommendations to funding panels where required.
Ensure public resources are allocated fairly, transparently, and in accordance with statutory duties.
Safeguarding and Risk Management
Identify and respond appropriately to safeguarding concerns.
Undertake safeguarding enquiries and contribute to safeguarding investigations where required.
Complete strengths-based risk assessments that balance safety with an individual's right to make informed choices.
Promote positive risk-taking and least restrictive approaches in accordance with the Mental Capacity Act.
Mental Capacity and Best Interests
Undertake Mental Capacity Assessments where required.
Support and facilitate Best Interest decision-making processes.
Ensure practice complies with the Mental Capacity Act 2005, associated Codes of Practice, and relevant case law.
