Pay rate to £35 per hour

Pertemps are recruiting for an experienced Adult’s Social Worker in the Scunthorpe area. 
This is to work in an Integrated Discharge Team 

• The Integrated Discharge Team is looking for a social worker to work a shift pattern over 7 days to complete Discharge Facilitation forms as par of the MDT team at Scunthorpe General Hospital. 

• Completed discharge facilitation forms.  

• Have an understanding of the care act, mental capacity act, chc checklists and how to commission services.
