Pertemps are recruiting for an experienced Adult’s Social Worker in Bournemouth. 
This is to work in a Hospital Team in Poole. 

• We are seeking an experienced Adult Social Worker to join the Hospital Social Work Team based at University Hospitals Dorset.  

• The post holder will undertake strengths-based assessments, support complex discharge planning, complete Care Act assessments, safeguarding enquiries, Mental Capacity Act work, and multi-disciplinary working to facilitate safe and timely hospital discharge.  

• Applicants must have experience of Adult Social Care and hospital discharge pathways.
