Head of Service Adult’s Social Work, Hospitals and Continuing Healthcare, Sussex 
Pay rate £650-700 per day

Pertemps are recruiting for an experienced Head of Service within Adult’s Social Work in East Sussex (Lewes area) 
This is to work in the Hospitals and Continuing Healthcare service 
Purpose of the Role  

• The role is responsible for the management of the ESCC ASCH Hospital Social Work Teams, Community Hospital Teams and the ASCH Continuing Healthcare Team (CHC) covering Brighton, Haywards Heath and as far as Maidstone. 

• The role is required to participate in regular system and system escalation calls and meetings for the health and social care economies, including some weekend and bank holiday calls during winter and periods of escalation. You will be a member of, and virtual and physical attendance, at the East Sussex  Operational Executive weekly meeting for both East Sussex and Brighton. 
  You will act as a single point of access for information, reporting and queries for all ASC services related to hospital discharges including services and will represent ESCC at various regular, time limited or one off forums associated with hospital discharges, i.e.Combined Urgent Care Operational Resilience Group (Brighton System), Winter Delivery team (ESHT system), NHSE Winter debriefing workshops (across systems). 

• The role is responsible for developing and implementing the ESCC elements of: 

• Winter Escalation Plan 

• Whole System Escalation Plan 

• Discharge to Assess /Home First 

• Be part of an out of hours duty rota as appropriate to the post. 

 

• Provide professional leadership on complex practice, safeguarding, and organisational challenges, supporting effective decision-making across Adult Social Care.  

Essential Requirements  

• Qualified Social Worker 

• Significant senior leadership experience within Adult Social Care at Head of Service level 

• Be able to attend the office in Lewes at least 1 day per week, and when needed
