The role provides an exciting opportunity to work in a multi-disciplinary setting, with a focus on strengths based practice and promoting independence and recovery. The main duties of the role include completing Care Act assessments and devising care and support plans, completing reviews, completing Mental Capacity Assessments, chairing Best Interests meetings, signposting, providing advice and information and working with people with complex social needs. There is also a Safeguarding function within the service, which includes completing initial fact finding and assessing and minimising risk. Our Intermediate Care Service (ICS) is seeking a Social Worker to work within the Reablement Service. This role offers a unique opportunity to thrive in a multi-disciplinary environment, collaborating closely with health professionals. We place a strong emphasis on strengths-based practice, crisis management, independence, and recovery, ensuring that each individual receives a comprehensive support tailored to their needs.
