This is to work in a Reablement and Hospital Discharge Team
- The Reablement and Hospital Discharge Team is committed to supporting individuals to recover and maintain their independence following a period of illness or injury.
- As a Deputy Team Manager, you will be integral to the team's development, meeting service demands, and implementing changes to support workers in maximizing individuals' independence. You will support the team manager in overseeing a busy, fast-paced team where every day is different. Implement creative solutions to help individuals maintain their independence.
- Collaborate with health professionals and continuing health care services.
- Lead the development of a supportive team environment. ·
- Provide reflective supervision and support to social workers and social work assistants. ·
- Manage and prioritize allocations based on risk, need, and urgency. ·
- Support workers in producing strengths-based assessments that accurately reflect the individual and their situation. ·
- Collaborate with health colleagues and other professionals to ensure timely discharge of individuals. Oversee the flow through all discharge services.
