Hospital Social Worker, Tower Hamlets 
The Hospital Social Work Team effects safe hospital discharges for Tower Hamlets patients - particularly using the Discharge to Assess Model – Home First Policy. The work is fast paced with a quick turnover of assessments and caseload. Our cases are transferred to the appropriate locality teams once patient is discharged safely therefore, we do not hold on to cases and we do not operate a waiting list system. We operate a hybrid system of working on site and from home. The team is very supportive with a strong management system. It is 35 hours a week role, Monday to Friday, 9am to 5pm and we are looking for a candidate who can start work immediately. Please note that Hospital Social Work D2A experience is a must. 
£32.53 per hour
