Senior Early Health Practitioner, Southwark
We are seeking an experienced practitioner to deliver high-quality family assessment and support for families in need of help. The role will champion best practice in whole-family work, using a systemically informed approach to drive sustainable change.
Key Responsibilities
Lead on Early Help: Work with families where children are at risk of poor outcomes, providing early intervention to address emerging issues, improve engagement, and prevent escalation to statutory services.
Support Complex Cases: Continue progress with families transitioning from statutory intervention, managing high levels of complexity and ensuring stability.
Act as Lead Professional: Assess the needs of all family members and develop a whole-family plan that promotes safeguarding, school attendance, and positive citizenship.
Direct Work with Families: Engage children and families in meaningful, sustainable change within available resources, encouraging positive parenting through a twin-track approach of high challenge and high support.
Outcome-Focused Practice: Drive improvements that safeguard children, strengthen family resilience, and reduce the need for higher-level interventions.
