Social Worker, Adults (West Suffolk hospital)
£35ph
West Suffolk hospital - 3 days office 2 at home Working under care act, completing mental capacity assessments, best interests’ decisions, adult safeguarding enquiries Assess patient needs – identify social care, safeguarding, mental health, housing, and support needs for adults being treated in hospital. Coordinate safe discharge planning – work with healthcare teams, families, and community services to ensure patients can leave hospital safely with the right support in place. Arrange care packages – organise home care, residential placements, rehabilitation services, or specialist support as needed. Advocate for patients – ensure individuals’ wishes, rights, and best interests are represented in care decisions. Provide information and guidance – support patients and families with advice on benefits, support services, equipment, and community resources. Safeguard vulnerable adults – identify and respond to concerns about neglect, abuse, or risks to wellbeing, following safeguarding protocols. Work in multidisciplinary teams – collaborate with nurses, doctors, therapists, and community agencies to deliver joined up care.
