Family Wellbeing Practitioner, Merton 
£25.70 per hour.  
The Family Wellbeing case practitioner  will take on the role of lead professional, engaging intensively with a caseload of 12-15 families who have multiple and complex needs and  leading / coordinating support services across a multi-agency network of professionals within  both children and adult services with the aim of reducing the full range of risk factors that may lead to developmental delays, statutory social care involvement, high risk behaviour, non-attendance at school for children and other vulnerabilities as described in the eligibility criteria for access to the service.
