Social Worker - Learning Disability Partnership, Cambridgeshire 
Pay rate - £38ph  
High cost review project- Preferably experience in assessments and care and support planning in line with the Care Act 2014, working alongside brokerage and contracts, evidence based practice and reviewing of provider documentation, good organisation and communication skills, must be able to travel countywide and have a full drivers licence. Currently 6 weeks- with aim for extension for longer term work County wide, however main office base – Cambridgeshire County Council Learning Disability Team Sunley House Summer’s Hill Drive Papworth Everard CB23 3AB Will need to be able to travel around Cambridgeshire for visits. No set office days currently, however scope this will change to 1 day a week in the office base
