Job Description
Adult Advanced Practitioner, Nottinghamshire
£37 per hour
Experienced, passionate and dedicated advanced social worker practitioner (ASWP) to join our Urgent and Emergency Care Team. The team covers the Acute and sub acute hospital in the South of Nottinghamshire (Queens Medical Centre, Nottingham City Hospital and Lings Bar Hospital).
The ASWP will need to have experience and a good understanding of the Care Act 2014, the Mental Capacity Act 2005 and the Department of Health Discharge to Assess guidance. The ASWP will need to have experience and understanding of the various pathways people may experience when leaving hospital. The ASWP will need to understand the impact that any delayed transfer of care (DTOC) has on the person and system. The ASWP will need to be able to demonstrate proactivity, partnership working and be an effective problem solver.
The ASWP will need to have line management experience and be prepared to support the management team to improve and drive up social work practice. The ASWP will need experience and understanding of owning and driving performance, coaching and strength based practice.
The ASWP will need to have good organisational skills, good communications skills, be able to make defensible decisions and work from their own initiative and be able to work well within a team environment. The ASWP will also need to attend and chair meeting with health colleagues and deputise for the Team Manager as and where required.
