Job Description
Adult’s Social Worker, Bromley
We are seeking an experienced and motivated Social Worker to support our Extra Care Housing Step-Down Pathway service, helping to manage winter pressures while undertaking strengths-based reviews for adults receiving care and support services.
This is an exciting opportunity to play a key role in ensuring residents receive the right support at the right time, promoting independence, wellbeing, choice and control while supporting timely hospital discharge and preventing unnecessary admissions and readmissions.
Working collaboratively with residents, families, providers, health colleagues and partner agencies, you will:
Facilitate safe, timely and effective hospital discharges.
Oversee and manage the Step-Down Pathway, supporting residents through recovery and reablement-focused interventions.
Undertake strengths-based reviews and reassessments to ensure support arrangements remain proportionate, outcome-focused and aligned to people's current needs.
£32 per hour
