Commissioning Team
Rate: up to £27.12 per hour
 
We are recruiting for motivated worker to join our busy team. This role is with one of the UK’s highest performing local authorities, serving as one of the best places to live and work in.
 
You must have relevant experience in a similar role; local authority experience is desired.
 
The post-holder will be part of the Commissioning Team and work closely with Service Managers, Senior Practitioners and Care Managers to source, arrange and amend packages of care for placements in residential/nursing homes and supported living services. This may involve regularly dealing with complex and/or contentious issues.
 
Experience of hospitl discharges and knowledge of the Mosaic system would be an advantage.
