Vitalis are recruiting for an experienced Social Worker to work in a Neighbourhood Team in Bolton Council.
We are seeking an experienced and motivated Adults Social Worker on a full time basis: 37 hours per week over 5 days to work on our neighbourhood team in the west & central north area of Bolton (BL1, BL5, BL6)
The role requires you to work within a person-centred service, supporting adults with a wide range of needs to achieve positive outcomes and maximise their independence.
You will have experience undertaking Care Act Assessments, developing support plans, facilitating care arrangements, and working collaboratively with service users, carers, and partner agencies. You will play a key role in safeguarding adults, managing risk, and ensuring individuals receive the right support at the right time.
As part of this role, you will contribute to the team's duty function and work within a multidisciplinary environment to deliver seamless and effective services.
