Qualified Social Worker, Hospital Discharge Team, Manchester City Council
Pay rate to £30 per hour
Vitalis are recruiting for an experienced Social Worker to work in a Hospital Discharge Team in Manchester City Council.
We are looking for motivated, confident and experienced Social Workers in a fast paced MDT team working within the HDTA Hospital Service for Central Manchester.
This is a busy and focused team which involves critical thinking and the ability to manage risk and need in partnership with Health colleagues within the HDTA service. The role involves assessment and support planning for citizens who live in Central Manchester and are inpatients at hospital, and people who have moved from hospital in to bedded care placements. Social workers will be required to undertake Mental Capacity Act Assessments and coordinate Best Interest Decisions. Social workers will be required to lead on Safeguarding Investigations. Social workers will be required to be part of the team duty rota.
The role will suit Social Workers able to work at pace in collaboration with others to support citizens discharged from Hospital under the HDTA service.
