Support Worker

As a Community Support Worker, you will support adults and young individuals with a range of needs, including learning disabilities, autism, mental health needs, physical disabilities, and older people living in the community.
You will build positive relationships with the people you support, enabling them to achieve their personal goals and participate fully in their communities.
Key Responsibilities

• Provide person-centred support tailored to individual needs and preferences.
• Assist with daily living activities, including personal care where required.
• Support individuals to access community activities, appointments, education, employment, and social opportunities.
• Promote independence, choice, dignity, and inclusion.
• Assist with medication administration in line with policies and procedures.
• Maintain accurate records and reports.
• Follow safeguarding, health and safety, and confidentiality requirements.
• Work collaboratively with families, healthcare professionals, and colleagues.
