Job Overview: As a Band 5 Clinical Pharmacy Technician you will work within the Pharmacy Department at this busy NHS Trust, supporting delivery of safe, effective, and patient-centred medicines optimisation services. You will work closely with pharmacists and the wider multidisciplinary team to provide high-quality clinical pharmacy support across inpatient areas, contributing to improved patient outcomes and the efficient use of medicines.
Key Responsibilities: - Undertake medicines reconciliation for patients on admission, transfer and discharge
- Support Ward-Based clinical pharmacy services alongside pharmacists
- Work collaboratively with the multidisciplinary team to optimise medicines use
- Provide accurate medicines-related information to patients and healthcare professionals
- Ensure compliance with local policies, national guidance, and NHS Standards
- Contribute to service development, audit, and quality improvement initiatives
- Maintain accurate clinical and pharmacy documentation
What you’ll bring to the role: - GPhC Registration
- CPT Qualification
- Ability to work independently and as part of a team
- Demonstrate a commitment to patient safety and continuous improvement
- Have good communication and organisational skills
