Job Description
Your primary focus is on ensuring the well-being and safety of older adults by assessing needs, developing care plans, and connecting them with essential services and support systems.
Your role includes:
- Assessment and Care Planning
- Service Coordination and Advocacy
- Collaboration and Communication
