💼 Key Responsibilities
In this role, you will:
- Conduct comprehensive assessments of adults’ care and support needs
- Coordinate safe and timely hospital discharges in partnership with NHS colleagues
- Manage a varied caseload with autonomy and professionalism
- Develop individualised care plans that promote independence, dignity, and empowerment
- Work in line with current legislation and best practice in adult social care
- Build trusted relationships with service users, carers, and multi-agency teams
- Offer support and guidance to unqualified staff and students, helping to build capability across the team

We're looking for someone who is:
- A Qualified Social Worker registered with Social Work England
- Experienced in adult social care, particularly in hospital discharge or community settings
- Skilled in risk assessment, care planning, and multi-disciplinary collaboration
- Knowledgeable in relevant legislation and frameworks
- A clear communicator, problem solver, and advocate for vulnerable adults
- Proactive, compassionate, and committed to achieving positive, sustainable outcomes
