This frontline service carries out a unique and fast‑paced function within Adult Social Care, offering exposure to a wide variety of cases — including some that are unusual or rarely encountered elsewhere. About the RoleAs part of the Assessment & Triage Team, you will provide professional triage and assessment at the front door of Derbyshire Adult Care. You’ll be supporting adults with a broad mix of needs, disabilities and circumstances, completing assessments within short timescales and helping people access the right support quickly.
