Social Care Assessor, Enfield
£23.87 per hour.
We are seeking a motivated and enthusiastic Social Care Assessor to join the Integrated Learning Disabilities Service (ILDS), supporting young people with learning disabilities as they transition from Children's Services into Adult Social Care. Working alongside the Senior Practitioner leading our Transition Project, you will undertake assessments, develop person-centred support plans, review care packages, and work closely with young people, families, schools, health professionals and partner agencies to ensure successful transitions into adulthood. The role will also involve safeguarding work, participation in duty, and working within a strengths-based, person-centred framework. We are looking for individuals who are passionate about working with people with learning disabilities and their families. If you are committed to supporting people to live autonomous lives, achieve positive wellbeing and reach their full potential, we would love to hear from you. ILDS is a fully integrated health and social care service comprising Social Workers, Community Nurses, Psychologists, Occupational Therapists, Speech and Language Therapists, Physiotherapists, Psychiatrists and other specialist professionals. The original 3-month contract has a potential to lead to a longer-term fixed-term appointment, subject to service needs and project review. What we offer: An exciting opportunity to be part of a dedicated Transition Project. Flexible hybrid working – 3 days office-based and 2 days working from home. Wednesday is a mandatory office day for the team. Supportive management and opportunities for professional development.
