Adult Social Worker – Community Team
Location: West London
Pay rate: £34–£36 per hour
Contract: Locum
Team: Community Adult Social Care Team

Pertemps Social Care are currently recruiting for an experienced Adult Social Worker to join a Community Adult Social Care Team in West London.
The successful candidate will work with adults in the community, completing assessments, developing care and support plans, and working with individuals, families and partner agencies to promote independence and positive outcomes.

Key Responsibilities

• Complete Care Act assessments and reviews.
• Develop and review care and support plans.
• Safeguard adults and identify and manage risks.
• Promote independence, choice and wellbeing.
• Work collaboratively with families and multi-agency professionals.
• Maintain accurate and up-to-date case records.

Requirements

• Qualified Social Worker with current Social Work England registration.
• Experience within Adult Social Care.
• Knowledge of the Care Act 2014 and safeguarding procedures.
• Strong assessment, communication and case management skills.
